Requirements For Tryouts and Being On The Team

In order to be allowed to try out for the team and also a requirement to practice and play on the team, the following must be completed

The Clearance Process

All students (including eighth-grade students entering high school in the fall) must be cleared to participate in any sport, band, cheer/song, dance team and vocal ensemble/choir. 

 

Clearance requires completion of the following four items.  Please see below for an explanation of each one:

 

1.  2023-2024 Athletic Registration" (online)

2.  2023-2024 DMV/Risk Management School Driver Registration" (online)

3.  Physical (print form below and turn in at school front office) 

4.  Copy of front and back of health insurance card (turn in at school front office). 

 

Your student WILL NOT be allowed to try-out or participate until they are cleared.

1. Online Athletic Registration

To complete the online athletic registration, please print the "2023-2024 Family ID Registration Instructions" below and then click on this link:  
https://www.familyid.com/yorba-linda-high-school 
Once you have submitted the online registration, you should receive an email confirming the registration and a notification will be sent to the school.  You do not need to print out or turn in anything from the website. 

2. Online DMV Registration

To complete the online DMV registration, please use the same instructions in #1 above and click the blue link above for the Family ID website.  As with the athletic registration, once you have submitted the DMV registration, you should receive an email confirming the registration and a notification will be sent to the school.  You do not need to print out or turn in anything from the website.

3. Physical (Turned in to the front office)

THE PHYSICAL MUST BE DONE ON THE DISTRICT FORM  or it will not be accepted.  Do not turn the physical in to the coach. Physicals are good for one year from the date of the physical.  If you think your physical is current, please contact Mrs. Belleville and let her know (contact info below).

Physical examinations for summer and for the season are being given in the gym at Esperanza High School on Wednesday, May 24th, 2023 4-8pm (doors close at 7pm) for $35

4. Health Insurance Card

At the beginning of each school year, you must provide a copy of the front and back of your health insurance card, even if it is the same insurance that you had the prior year.  Do not turn the insurance card in to the coach.  You cannot participate without health insurance.  If you do not have insurance, you can obtain it for school athletics through Myers-Stevens.
